This page includes the form and instructions for reporting select data tables regarding local district special education programs. Specifically included are those data tables required for the 2006-2007 End of Year Data submission including the Special Education Exiting and Discipline Data Tables. This year's version of these data tables changes from an aggregate reporting of district totals to separate student lists for the various data needs.
The form below is to be completed by July 31, 2007 and covers data on special education students throughout the 2006-2007 school year relative to their exiting special education and disciplinary removals from their special education placement.
Please note there are significant changes in this year's data reporting so be sure to read the instructions document. The completed data file is no longer to be emailed to the Department of Education as in past years due to the confidentiality of the data contained. The accompanying instructions include the process for submitting the data file through a secure upload process.
