Documents for directors and Head Start coordinators who work with the Head Start Program. Documents are either downloadable below or we have provided a link to an outside website to assist you. Documents include Local Agreement on Full Utilization of Head Start, Full Utilization of Head Start Spring Documentation, Full Utilization of Head Start Certification, Official Head Start Count for Kentucky Preschool and Contracting Application forms.

The Local Agreement on Full Utilization of Head Start This agreement is required as part of your district’s consolidated planning process. Upon completion copies are kept locally.
The Full Utilization of Head Start Spring Documentation This form is used to document communication between local districts and Head Start Grantees. Upon completion copies are also kept locally.
The Full Utilization of Head Start Certification This form is a documentation of the Full Utilization of Head Start programs as of September 4, 2007 and must be submitted to the Kentucky State Head Start Collaboration Office by September 14, 2007.
The Official Head Start Count for Kentucky Preschool This informational document provides "Pre-KERA" Head Start enrollment numbers of 4s and assists Preschool/Head Start partners in determining their targeted Head Start enrolled 4s number.
Kentucky Partnership Agreement This Partnership agreement between Kentucky Department of Education and the Region IV Head Start Bureau is designed to enable Head Start Grantees and Local Education Agencies to maximize their potential by sharing and coordinating local, state, and federal resources. This document serves as a guide for the Head Start Grantees and Local Educational Agency as they develop their local agreements.
