The Local Agreement on Full Utilization of Head Start
This agreement is required as part of your district’s consolidated planning process. Upon completion copies are kept locally.

This form is a documentation of the Full Utilization of Head Start programs as of September 4, 2007 and must be submitted to the Kentucky State Head Start Collaboration Office by September 14, 2007.
