Off-List Notifcation - and Forms

A school council or principal must notify, through the superintendent, the State Textbook Commission that it plans to adopt a basal textbook or program that is not recommended by submitting evidence that the title they have chosen meets the selection criteria of the State Textbook Commission.  KRS 156.445 (2)

Please fill out the Off-List Notification form and return it to the Kentucky Department of Education.
